You have already taken the first step! The next step is to call our office (650) 229-1010 and schedule a new patient appointment. Our receptionist will be happy to answer any questions you may have.

You will be asked to fill out some forms that you can download (below) or have emailed to you. If you do not have access to a computer, we can mail or fax them.

New patient forms:

• Acknowledgement of receipt of privacy issues-PDF

We request that you fill out these forms and return them to us before your appointment. It is also helpful to send us past lab test results from the past 5 years and any other test results (ultrasound, MRI, CT scans, X-rays, etc.). An authorization for release of medical records is necessary to get these records (see link below). When we receive them before your appointment, Dr. Metzger has a chance to review them, which allows you to get the most out of your appointment. We will still see you if you bring the papers with you or even if you have not filled them out.

We do offer telephone appointments for new patients, which are particularly helpful if you are planning on traveling here from out of the area. Dr. Metzger is unable to prescribe medications until you are seen in the office. For patients who want a second opinion, consultation, or coordination of care, this can be done exclusively by phone.
